states. Upon completion, you will receive a promotional
discount code for up to $50 on any NDAS book or
software.

The online survey form will prompt you for your
standard/list price per dental code. Do not provide
contracted fees or medicaid pricing. Survey participants
remain anonymous but a zip code is required.
